Enrolling Your Child
In order to enroll your child in Mitchell Elementary School, you must see our school secretary (203) 263-4314.
You will need to complete:
- REGION 14 SCHOOL DISTRICT STUDENT REGISTRATION FORM
- SIGNED RELEASE OF INFORMATION FORM
- Academic Records from previous school
- Medical Records from previous school
*** PLEASE NOTE THAT NO STUDENT WILL BE ADMITTED TO MITCHELL ELEMENTARY SCHOOL UNTIL MEDICAL RECORDS ARE RECEIVED.
- PROOF OF RESIDENCY
- Rental Contract
- Mortgage Statement
- Utility Bill (cable, gas, or electric)
- Driver's License
- BIRTH CERTIFICATE
